You will need:

You will receive an email to connect to the virtual visit. Click on the link and follow the prompt to download the zoom app. (check your spam folder.)

Cost is $55/visit and covered by some insurances. If you'd like we will bill your insurance after the appointment and reimburse you whatever they cover.
